A Clinical Study for the Treatment of the Tuberculesis Empyema


Abstract
A clinical review was madeon the 64 cases of tuberculous empyema treated at the 15th Army Hospital from Jan. 1. 1966 to Dec. 31. 1968.
The results were as follows;
1. In the 64 cases of tuberculous empyema, 50 per cent of the cases were only developed without the other tuberculous lesion and 34.4per cent of the cases were combined with pulmonary tuberculosis. The rest 15.6 per cent were combined with extrapulmonary tuberculous lesions.
2. Although the early cases of localized empyema were cured of the disease with closed thoracotomy, the other chronic types of empyema were mostly failed to cure with thoracentesis and closed thoracotomy.
3. In the chronic empyema, the decortication was the treatment of choice, however the 27.5 per cent of the cases were failed to cure because of poorexpansion of the lung following decoration,
4. In combined cases with pulmonary tuberculosis the decortication and the pulmonary resection were usually indicated and the pleuropneumonectomy was indicted in the cases of more severe or extensive pulmonary tuberculous lesions.
5. It seemed that the thoracoplasty was of little value in the surgical treatment of chronic empyema because of postoperative complications such as skin fistulas in our cases.
